ENROLMENT FORM

We ask everyone who attends workshops, in-person classes, retreats, online classes and courses to complete an enrolment form. This asks for your name, address email and phone numbers and has a large health information section.

To be able to do our job as a Pilates teachers, Franklin method educators, UHP practitioners and Dance teachers, we need to ask you information about health conditions and injuries. It is very important you give accurate and current information, and keep us up to date with your state of health.

The only people with access to your information is Rachael Hall Ltd’s administrator and team of teachers.

Screening forms are stored securely for 7 years as either paper copies in a locked filing cabinet or password protected online. We do not discuss teaching you or any details of your health with anyone outside of Rachael Hall International LTD.

We may ask permission to talk or email your physiotherapist, consultant, doctor or other health professional if we think this would benefit you. You would be entitled to see this correspondence if requested.
